This book seems to be good, but it seems kind of short, especially compared to kaplan. Its only 250 pages and about the last 50 pages are images. Page 10-37 is pediatrics and 37- 122 internal medicine, then 20 pages of gynecology and obstretics, etc. Here is what is says on
"Biliary tract disease Jaundice can also be caused by bile duct oabstruction. Look for marked elevation of alkaline phosphatase (or gamma-glutamyl transpeptidase (GGT) or 5' nucletidase). Conjugated (i.e., direct) bilirubin is more elevated than unconjugated bilirubin. Other features include pruitus, clay colored stools, and dark urine, which is strongly bibirubin positive. Unconjugated bilirubin is not excreted in the urine becuase it is tightly bound to albumin.
1. Common bile duct obstruction with gallstones (ie choledochithiasis): Look for a history of gallstones or the four Fs (female, forty, fertile, fat) and abdominal pain in the right upper quadrant. Ultrasound may be able to image the stone: if not, use magnetic resonance cholangiopancreatogramy (MRCP) to diagnose and endoscopic retrograde cholagiopancreatography (ERCP) to treat (FIg. 3-25)
2. Common bile duct obstruction from cancer: ussually pancreatic cancer (painless jaundice and papable gallbadder= Courvoisures sign), occasionally cholangiocarcinoma or bowel cancer.
3. Cholestasis: often from medication (birth control pills, phenohizines, androgens) or pregnacy.
4. Primary biliary cirrhosis: middle - aged woman with no risk factors for liver or biliary disease, marked pruitus, jaundice, and positive antimitochondrial antibodies (the rest of the workup is negative). Cholestryamine helps with symptoms, but no treatment is available other than liver transplantion.
5. Primary sclerosing cholangitis: seen in young adults with ulcerative colitis; manifests as with infectious cholangitis.
6. Cholangitis: infected bile, usually under presuure. Treat with antibiotics, and remove any stones/ obstruction surgiclly, percutanously, or endoscopically.
Case Scenario: a 39 year old woman with a history of gallstones develops right upper quadrant pain, chills, and jaundice. What happened? An impacted gallstone in the common bile duct has led to cholangitis. Charcots triad of cholangitis is jaundice, fever and right upper quadrant pain.
Thats it, and then it goes onto Inflammatory bowel diesease for 2/3s of a page. It seems to be set up like that, for Liver Disease, it breaks it down into acute liver disease, and goes through the causes, and then does the chronic liver disease and the symptoms and causes (hemochroatosis, wilsons, alpha antitrypesin definceincy).
I do like it so far though, it is very dense. Tells you what seems to be important material. Hope that helps sorry for the spelling.

Can serve as an adjunct for quick review.

Had purchased this and some other books for further prep after I sat 2CK. This isn't great but it's not bad either. I haven't picked up on errors and it seems to be fairly clean. The only reason I give it three stars is because the content is non-sophisticated (not un-) and doesn't add anything special to what I already knew. However there is a good section on herbs in medicine and their uses/side-effects; it's not that we would ever recommend these to patients, but the section serves for us to know how they relate to one's presentation if a patient mentions one.
What I can say outright is that you'll be bored reading this text if you're fresh out of Step 2CK because the content is fairly basic and easy. *However*, if you haven't touched the material in a while and need a good and CONICISE review, this is definitely a text that one could consider.

Fine for information

Fine for information, but you don't really need this for step 3- --I advise doing as many questions as possible, you don't need to read anything really. Answers to the test itself are not easy to take from material in books like this, it's more about judgement and management and less about information, if that makes sense
